HISTORY OF PRESENT ILLNESS: heroin abuse for the last three months

PAST MEDICAL HISTORY: History of heroin and narcotic abuse from 1977 - 1980, went through methadone treatment and has been 'clean' until early March of 2010. Also treated sporadically for depression; therapy and psychiatric treatment, but has never been hospitalized or treated outpatient for mental issues.
Father committed suicide when patient was 19, history of depression.

SOCIAL HISTORY: Born in Wiltshire, England, UK and grew up lower middle class. His father owned a shoe shop that did moderate business, and his mother was a secondary school piano teacher. He had a younger brother five years his junior, Ezekiel. Both his parents were withdrawn and quiet, and while they never argued around him or scolded him harshly, they rarely showed affection or praised achievements. His father committed suicide by hanging in the small back room of his shop. Both Gabriel and his younger brother Ezekiel found his father, as they helped out around the shop after school most afternoons. Both men were psycholgically and emotionally scarred by this experience, moreso Ezekiel who became catatonic for four months afterward.

Gabriel went on to university, graduating with honors in English Literature and Music, then moved to New York to find work in a small publishing firm. He wrote in his off hours and eventually would finish his first novel, a story of a teenager's coming of age and his subsequent growth into manhood and the struggles he had to overcome. Never a bestseller, it was critically praised and would become a minor cult classic. He has written four other novels since, and published several chapbooks of poetry.

During his first years in America, Gabriel struggled with his depression and turned to drugs, working his way up to heroin to alleviate his pain. It would threaten to destroy him, but it would later give him inspiration to write his first novel, which he started writing as he was being treated for his addiction at a methadone clinic.

Soon after he'd kicked the habit, he'd turned 23. That year he would meet Ophelia Phillips, the woman who would eventually become his wife. She was also poet, and he met her at a reading at a coffee shop. They talked through the night, and he went home with her. One thing led to another and they fell in love, forming a deep bond that would last until she died. In 2008 Ophelia was diagnosed with Leukemia, and two years later in January of 2010 she died peacefully at home in the company of her sister and her husband.

After she died, Gabriel lost track of his life. He returned to his old habit of heroin, living off the royalties from his past novels and rarely leaving the house but to purchase drugs. He ate little, slept constantly, and lost the hunger for life he once had when Ophelia was alive. His Ezekiel came to visit and was shocked by Gabriel's appearance. He would soon find out why his brother was so sickly and thin when he misjudged a hit and Zeke found him in the bathroom, victim of overdose.


LIVING WILL: If I should become incapacitated and unable to make my own decisions due to coma, mental or physical disabilities leaving me unable to communicate my wishes, I appoint my brother, Ezekiel Brian Samedi to be my agent, and to have the power to make such decisions regarding my life.
