HISTORY OF PRESENT ILLNESS: Jeremiah "turns into" multiple people that effect his everyday life. Was placed at Bayview to keep him safe and not be in danger in the real world. Their mother died from Cancer and he was under his sister's care until she became pregnant and could no handle her brother and her family at the same time. Jeremiah also goes by Tony Felicioni (A mobster), Jimmy Barnes (a janitor), Dr. Timothy Concave (a Neurologist) and Marty (a friendly nudist)
PAST MEDICAL HISTORY: There have not been any history of mental disorder's in the family. His mother was the only case of cancer as well.
SOCIAL HISTORY: Jeremiah grew up in a loving household with his mother and older sister while his father had never been in the picture. He lived in Boston, Massachusetts and his first sign's of his disorder were easily over looked during his younger years. His mother dismissed them as him pretending to be different people in a playful manner. However, by the time he reached his teen's it was no longer considered for play and he was getting into trouble in school. It even went as far as Jeremiah pretending to be the Principal of the school and announcing a day off over the intercom. After this, his mother decided it was time to get him checked out and at the age of 16 he was diagnosed with Dissociative Identity Disorder. He began to take medicine's to tame it and while he did calm down a bit, it still was not a significant amount.
His mother devoted her life to taking care of Jeremiah and watching him, beginning to work out of their home. His siter Jessica went off to NYU while Jeremiah receieved special care at his school. Jessica and Jeremiah's mother was diagnosed with breast cancer when Jeremiah was 22 years of age and six months later she passed away. She never got herself checked out because her mind was preoccupied with her baby boy's disease. That was when Jessica, who had married and had a little boy of her own, took her brother into her home and became his primary caregiver. Though it seems since his mother's passing, his split personalities have begun acting up severly and Jessica has found that it is becoming more difficult to take care of house and her brother.
